What I did wrong: I had taken modafinil for about two months without issues. Then I took it after a poor night of sleep (4 hours), combined it with 3 large coffees because I had a critical deadline, added a pre-workout supplement that contained yohimbine and synephrine, and took 400 mg (double my usual dose) because I felt I needed more.
What happened: About 90 minutes in I developed a racing heart (HR 140+ bpm at rest), significant chest tightness, sweating, and a feeling that something was seriously wrong. I went to the ER.
The ER diagnosis: Adrenergic overstimulation / hyperadrenergic episode. Not a heart attack. Resolved within 4 hours with monitoring. No permanent damage. Very frightening, very expensive lesson.
What caused it: The combination of modafinil (norepinephrine), caffeine (adenosine antagonism + catecholamines), yohimbine (alpha-2 antagonist, potent adrenergic), synephrine (beta-agonist), and sleep deprivation (elevated cortisol) created a massively additive adrenergic load.
The lesson: Never stack adrenergic compounds. Modafinil + caffeine is already two adrenergic inputs. Adding more is foolish. Especially do not do it on sleep deprivation, which compounds everything.
For the community: what pre-workout ingredients are safe to combine with modafinil if any? I want to understand where the line is.
Caffeine-free pre-workouts with creatine, beta-alanine, and citrulline are generally fine. Avoid: yohimbine, synephrine, DMHA, DMAA, high-dose caffeine, or anything marketed as an “extreme energy” formula.
The double-dose on top of everything else is what would have pushed anyone over the edge. Modafinil dose-doubling is rarely beneficial and always riskier.
This should be required reading for anyone using modafinil alongside exercise supplements. The supplement industry has many potent adrenergic compounds hiding in proprietary blends.

The sleep deprivation component is underappreciated. Poor sleep elevates cortisol and baseline adrenergic tone, making you more sensitive to every stimulatory input. Taking modafinil after a bad night is not the smart use case people think it is.
